Thank you for deciding to enrol your child into Yew Chung International School of Chongqing. The following items must be completed and received by the Admissions Office before an application can be processed. Documents can be submitted in person or by fax, email or mail using the information listed below.

YCIS Chongqing Student Application Form (completed in full, signed and dated)

YCIS Chongqing Student Medical History Form (completed in full, signed and dated)

Copies of school academic reports/records for the last (most recent) two years if the applicant is applying for Years 2–13 (For Year 1 applicants and below, please include any school reports/records, if available). These reports/records must be either in English or Chinese, or translated into English or Chinese by an authorized translation agency. Please also include any other documentation that may help the school to determine the appropriate programme for your child.

Payment of the Application Fee of RMB1,230 by:

Cash (in RMB only); or

Cheque (in RMB only); or

Four passport-sized photos for each child with the passport name of the child neatly printed on the back of each photo

Photocopies of the passport and visa page of both the child and the parents

(a)	Hong Kong and Macau residents: Photocopies of the
	i	Hong Kong Permanent Identity Card,
	ii	passport, and
	iii	《Taiwan, Hong Kong and Macau Staff Employment Permits》are required.
(b)	Taiwan residents: photocopies of the
	i	Mainland Travel Permits for Taiwan Residents, and
	ii	《Taiwan, Hong Kong and Macau Staff Employment Permits》are required.
